Student Mask Exemption Form
The student mask exemption policy for the 2021-2022 has been updated.  Please review the mask exemption form closely.
Change in Mode of Learning Request Form
Starting on Thursday, September 9, 2021, families and students wishing to change the mode of learning will need to complete and submit this form to the school principal or vice-principal.
Exemption from Synchronous Learning
Families requesting an exemption from synchronous learning must complete the form below and sending it to the Teacher.
